What it means: IDH1 mutations produce 2-hydroxyglutarate, an oncometabolite that blocks cell differentiation. Found in AML and gliomas.
Why it matters: IDH1 inhibitors (ivosidenib) induce differentiation and show responses in IDH1-mutant AML and cholangiocarcinoma.
Testing: Tested via NGS or PCR. R132 mutations are most common. Also detectable via immunohistochemistry.
